D1.Exchange
D1.Exchange is an Intelligent Connectivity Portal with Integrated Clinical Decision Support. This state-of-the-art portal has been designed to allow quick and easy connectivity and the exchange of data between all the healthcare stakeholders in a region. D1.Exchange provides:
1. Web Portals delivering rich personalized content to all participants.
2. Authenticated, authorized and role based online access for users to manage user profiles, organizational profiles, communication preferences, patient records and transmitted reports.
3. Code Management module that allows for the creation, maintenance and mapping of various local codes into other codes for tests, observations, results and clinical vocabulary etc. The module relies on long established coding standards such as LOINC, CPT, SNOMED (CT), and HL7 vocabulary. The module allows for the mapping of diagnosis, procedure, test, result and vocabulary local codes maintained in the various hospital, physician, pharmacy, payer and lab systems to other local codes or to standards based ones that the
public health agencies require. The code maps
are customizable and personalized to each system.
4. An Integration Management Module that facilitates the integration of the D1.Exchange system to the various hospital, physician, pharmacy, payer, lab and public health systems. The module supports

. Bi-directional communication based on various formats and communication protocols.
. Several HL7 message formats, both XML and EDI encoded, and custom defined message formats. In addition, the module
. Bi-directional communication using web services, ebXML, FTP, ODBC and JDBC data sources and manual upload and download of message batch files.
. The integration module is also designed to gather reportable disease information from the various data sources and systems to create the necessary reports for public health reporting. |
5. D1.Exchange performs online and background parsing, processing and error reporting of received data to facilitate the automatic transmission of data between participants.
6. D1.Exchange has been designed to support public health authority reporting capabilities including the ability to report data to state level departments of public health and the Centers for Disease Control and Prevention (CDC).
7. D1.Exchange has the option to add an Integrated Rules Engine that provides the ability to create and maintain rules that could trigger queries against external systems to obtain additional required information and to generate alerts based on this information, if required.
8. D1.Exchange provides a state of the art federated Patient Master Index. This technology provides numerous capabilities including:

Clinical Decision Support
D1.Exchange can be integrated with D1.Pathway our Clinical Decision Support product. Operating from within D1.Excahnge D1.Pathway can access relevant patient data from any of the systems connected to D1.Excahange. Based upon this data D1.Pathway can issue alerts and recommendations to care providers. This technology has been shown to reduce errors and costs significantly. Recent studies have shown reductions of 20% in hospital admissions for patients of care providers who had access to the information provided by D1.Pathway.
RHIOs and Revenue Generation
A significant challenge faced by RHIOs is how to generate revenue. While it will likely take a while for RHIO operators to create a sustainable business model; one promising opportunity is to provide Clinical Decision Support (CDS) capabilities to care providers using the various EMR and EHR systems connected to the RHIO. As payers provide incentives to care providers to achieve quality of care improvements, providers will be forced to acquire CDS technologies and as a trusted partner RHIOs will be in an ideal position to provide them access to this technology.
